Position Description: To provide an individual, with an Intellectual or Developmental Disability, individual services and support including care, supervision, support and teaching in the home of the family.
Daily rate: $61.00-$245.00 per day Position Duties: • Monitor the Customer’s overall health and safety needs and notify ST, INC Program Director(s) of any significant medical changes, illnesses, or emergencies. The Provider shall submit an incident report to ST, INC within 24 hours of the occurrence of such incident. • Always Comply with all requirements of ST, INC’s Policies and Procedures; the Rules and Regulations of the Colorado Department of Public Health and Environment and the Colorado Department of Health Care Policy and Financing. • Be responsible for assisting the Customer in maintaining his/her medical needs, which includes:
- Arranging and attending all medical appointments, including regular annual examinations (physical, eye, etc.)
- Entering medical appointments into Therap, printing the consultation form and ensuring that the information is filled out by the physician, in its entirety.
- Ensuring all documentation is completed by physician and promptly returned to agency nurse including physician order forms, copy of any prescriptions, orders for further testing or follow up.
- Follow up on the recommendations of health visits. This shall include promptly notifying the Program Director of any health/ medical follow up that is needed and scheduling these visits promptly
- Receiving treatment plans from physicians prior to any cares by a dentist or optometrist/ ophthalmologist. This care would be considered not billable under state Medicaid and would require Prior Authorization Request units from State which requires:
- Treatment plans to be submitted to ST, INC Program Director prior to any treatments being completed.
- Follow up with Program Director to ensure that PAR units have been approved; and
- Appointments with physicians to ensure completion of treatment.
- Medication Administration as prescribed by a physician. The Provider will document all medications administered to the Customer on ST, INC forms at the time the medications are given.
- Any other medical appointments including any recommendations for specialists.
- Ensure that the Customer receives any medications prescribed by a physician as scheduled. All medications received by the Customer will be documented on a Medication Administration Record at the time the medications are given to the Customer. Over the counter medications must be preapproved by the Customer’s physician.
- Provide basic first aid and health care supplies that the Customer might need or use on a periodic basis such as:
- First aid supplies (Band-Aid’s, thermometers, gauze, etc.)
- Over the counter medications for pain relief (Aspirin, Tylenol, Ibuprofen), as ordered by the physician
- Other health care products, as ordered by a physician, such as PRN medications, lubricants, rubbing compounds, antacids, laxatives, stool softeners, or enemas
- Assure that any emergency medical treatment necessary will be made available and ST, INC Program Director(s) will be notified by such treatment within a reasonable time thereafter if the Program Director(s) was not notified prior to such treatment. The Provider shall submit an incident report to ST, INC within 24 hours of occurrence of such incident.
- Purchase and provide food and any nutritional supplements ordered by a physician necessary to meet the requirements of this Agreement.
- Support the Customer in maintaining adequate nutrition, which includes:
- Ensuring the Customer receives three balanced meals per day or nutritional supplement(s) as ordered by a physician. The Provider will ensure that the Customer follows any special diets ordered by a physician;
- Assistance with grocery shopping;
- Preparing nutritious meals; and
- As necessary, preparing an individualized nutrition plan, with the assistance of ST, INC and documenting any special diet supports.
- Develop, with the assistance of ST, INC, a safety plan to respond to any and all critical situations that may arise in the home or while supporting the Customer including, without limitation, natural disaster, fire, intruders, or other emergency according to the Health and Safety Assessment/ Plan.
- The Home shall be maintained as a sanitary a sanitary and safe dwelling.
- The Caregiver shall be responsible for routine household upkeep such as changing light bulbs, taking out trash, etc.
- Regularly include the Customer in activities of interest to the Customer, such as going to movies, going out to eat, going shopping, and other such activities. The Provider shall assist the Customer in accessing the community
- for shopping, social, recreational, leisure, and employment opportunities and any other community resources as needed. The Caregiver shall utilize community resources whenever possible as means of integration into the community.
- Coordinate transportation as needed to community activities and health/ dental care appointments. Such transportation costs shall be paid by the Caregiver.
- Provide learning experiences to the Customer in the areas of domestic and self-help skills, and assist the Customer in completion of daily home activities such as meal planning and preparation, household chores, setting an alarm clock, etc., when such assistance is needed or requested.
- Act as a role model for the Customer and provide emotional support, guidance, and assistance with decision making.
- Bring problems or issues to the attention of the Program Director(s) as they arise and work cooperatively with the Customer toward resolution.
- Maintain documentation of services and supports being provided to the Customer.
- Assist the Customer with personal purchases including, without limitations, clothing, and medications;
- As indicated by the Customer’s Individualized Service Plan or Individual Service and Support Plan, assist with money management;
- Ensure that all of Customer’s monies spent are documented on finance sheets and accompanied by a receipt; (only applies to those individuals ST, INC is representative payee for).
- Submit in writing to the appropriate Program Director all requests for monies for other needs; (only applies to those individuals ST, INC is representative payee for)
- Meet with ST, INC Program Director(s) to resolve any questions or discrepancy with the monies spent. The Provider understands that inappropriate expenditures must be repaid and may result in termination of this Agreement; (only applies to those individuals ST, INC is representative payee for)
- Track and submit all monies earned or received by the Customer to assist in maintaining Social Security and Medicaid eligibility (only for those individuals ST, INC is representative payee for if this is monitored by ST, INC per the PNF status of the person)
- The Caregiver shall have and keep in effect during the term of this Agreement, automobile and homeowners/renters insurance in the amount not less than the minimum required by ST, INC policy to cover the Home and any vehicle used to transport the Customer.
- The Provider shall allow access to ST, INC staff during reasonable times, on an announced or unannounced basis, for the purpose of monitoring the environment and records of the Customer.
- Attend all mandatory meetings such as IP staffing, IDT (interdisciplinary team) meetings, ST, INC trainings and regularly scheduled monthly meets, etc. as deemed necessary by ST, INC Administration, the Customer or his/her team members.
- Notify ST, INC staff immediately and directly of any all concerns regarding the Customer.
- Share all pertinent information related to the Customer, including information regarding the Customer’s day program/ employment with ST, INC, and shall provide reasonable access to such information to ST, INC staff as requested.
- Agree to participate in any meetings necessary for the benefit of the Customer.
- Agree to work with ST, INC and all ancillary service providers including, without limitations, transportation, resource coordination, mental health, and home health agency service providers in a manner that is cooperative and that does not jeopardize the Customer’s access to services.
- Meet with ST, INC staff on a regularly scheduled basis, but no less than quarterly, to review the services and supports being provided to the Customer. Such meetings shall include a review of required documentation of services and supports being provided, and a review of personal needs ledgers, balances, and receipts for Customers personal needs funds (if applicable).

COMPANION PROVIDER POSITION
We are seeking enthusiastic caregivers who are willing to provide care to an individual who cannot live independently in their community and doesn’t want to live in a communal setting. Host Home Caregivers share homes and lives with individuals with Developmental Disabilities (IDD). As a Live-In Caregiver, you move into the client’s home and provide them hands-on ST, and ongoing daily care. You can help someone who has some daily challenges and make a difference in their life as they become part of your family. The caregiver will have their living area and access to the kitchen and living areas.
TO QUALIFY FOR THIS AMAZING OPPORTUNITY
- Must have experience with providing personal care

What services does the RESULTS program offer to individuals with disabilities? RESULTS stands for Real Employment Skills You Learn To Succeed. This program assists individuals with disabilities conduct a career search, negotiate employment, and get the support needed to obtain and retain employment. PPCH works closely with the Colorado Department of Vocational Rehabilitation Services, and other referral sources, to help individuals with disabilities find the resources they need to become self-sufficient. Description: The primary focus of the Employment Consultant is to assist individuals with disabilities (aka career seeker) to obtain and maintain paid community integrated employment. An employment consultant is responsible for matching job applicants to a specific job that ideally suits their talents and interests. Employment Consultants provide on-going training and supports to career seekers within their chosen work settings. In addition, they assist career seekers with obtaining the maximum level of independence within their job. Meeting individually identified support goals for future employment opportunities is a high priority objective.
The Community Integrated Employment program supports two diverse career paths: Job Coaching and Job Development. Experience level, interest, skill and ongoing training/certification will support career development and opportunities in career specialization into positions that offer opportunities for greater responsibilities and wages.
Job Duties:
- Assists career seekers with intellectual or developmental disabilities (IDD) in learning and performing job duties at community employment sites. Supports participants with gaining independence in job tasks using job coaching strategies.
- Provide on-site training, instruction, supervision, and advocacy for individuals with developmental disabilities at community job sites. A minimum of sixty percent of time spent in the community supporting individuals in the workplace.
- Advocate for career seekers while assuring their health, safety, and welfare at all times.
- Provide basic orientation skills to individuals with a disability, around their work site and required work routes.
- Establishes a strong mentorship with career seekers by providing quality supports.
- Work with employers to understand the needs of the organization and how career seekers can fulfill those needs within their role.
- Work with career seekers to understand and meet the needs of the employer and job.
- Work with career seekers to understand, practice and fine-tune their skills.
- Demonstrate the ability to break down job tasks into small steps the career seeker can understand.
- Conduct mock interviews, support with resume development, administer assessments.
- Follow career seeker’s Service Plan and DVR authorizations .
- Ongoing communication with supervisor, team members and family members.
- Attendance to team meetings as needed.
- Provide accurate documentation of employment consultant and job development contact notes along with DVR progress reports.
- Be able to meet deadlines with completing written reports.
- Observe confidentiality of all PPCH clients and operations.
- Deals effectively with crisis situations.
- Provides good customer service and communicates with team members in timely and appropriate manner. This includes verbal, written and electronic communication.
- Works productively and cooperatively as a member of a team.
- Ability to understand the DVR billing process.
- Driving to different locations throughout the Denver Metro area.
- Must participate in Employment Certification within 6 months of hire.
Qualifications:
- General knowledge of the developmental disabilities system in Colorado.
- Demonstrated skill in working with adults with developmental disabilities.

- Effective written and verbal communication skills with individuals and groups at all professional levels.
- Ability to work effectively with various technology including word processing software, internet based databases, assistive technology, etc.
- Possession of a valid driver’s license and ability to meet PPCH driving requirements.
- This position requires regular use of a personal mobile device such as a smartphone or tablet.

Salary Range: $60,000.00 To 75,000.00 Annually
Position Description: Behavioral Health Therapists conduct a wide range of evaluations & assessments, therapeutic interventions (CBT, DBT, behavioral, etc), crisis intervention, and/or case management services across many environments which requires a high degree of independence. Therapists can expect to work with multiple referral sources such as Community Center Boards (CCBs), Program Approved Service Agencies (PASAs), Medicaid and other insurances, as well as individuals and families who privately seek out our services. Our therapists support individuals in their homes, workplaces, day programs, schools, and anywhere else that is therapeutically appropriate.
Therapists can expect to provide primarily: Therapists can expect to provide services in the community at the clients preference and our office in Aurora and as well as some virtual sessions.
Licensed or License Eligible: Master’s Degree Required. Licensed or license eligible preferred. We offer clinical supervision to assist with licensure.

Therapist Position Duties:
- a) provide intensive, integrated professional mental health services and treatment planning in outpatient and community based settings in accordance with Health Care Policy and Financing (HCPF) as well as Colorado Department of Public Health & Environment (CDPHE) rules and regulation;
- b) provide professional leadership in the determination of appropriate therapeutic approaches and methods;
- c) plan and implement outreach/education programs, workshops, and activities;
